Our goal is to help you return to living life pain free through physical therapy. If you have scheduled your initial physical therapy evaluation, thank you!

At LSTC it is important to us that you start out right. Your physical therapist will conduct your one-on-one evaluation in a private room to allow you to discuss your health concerns. During your evaluation he/she will determine your specific needs and set up a personalized plan of care just for you. It is important that you share your physical therapy goals so they can make sure that they incorporate them into your plan.

You will need to bring the following items to your evaluation appointment:

  • Your insurance information cards.
  • A photo ID – driver’s license, up-to-date passport, or state issued ID.
  • Your physician’s prescription for physical therapy (if you have one).
  • Complete your online, patient paperwork prior to arriving for your evaluation.

IMPORTANT PATIENT INFORMATION – PLEASE NOTE:  On the day of your evaluation we need you to arrive no less than 10-15 minutes before your appointment time so we can process all your paperwork and get you started with the physical therapist on time. We have set aside 1 hour for your evaluation. If you are late for your appointment or your paperwork is not filled out ahead of time, you will likely lose precious evaluation time. We will not be able to go beyond the originally scheduled hour as our therapists have time set aside for other patients after your hour is complete.
